This program will perform a quick eligibility check and begin the process of entering a new applicant.
Income limit information will pull over into this program based on the income limit information entered per family size.
The following information is pulled:
Low Income Limit
Very Low Income Limit
Extremely Low Income Limit
Income Class
Click this button to clear the screen in preparation for re entering income information.
Select a county code for the eligibility check, if applicable. If your housing authority does not use County Codes, this field may be left blank.
Enter or increment to the number of family members who will be living in the unit.
Place a checkmark within the box, if the applicant is considered as an elderly or disabled family.
Enter or increment to the number of members within the applicant family who are under the age of 18 or over the age of 18, but full time students.
Enter the dollar amount of assets for the applicant.
For asset amounts of $5000 or higher the program will enter the amount of income that is imputed. User entry is disabled.
Enter the amount of income generated by the assets.
Enter the total of income from all other sources.
The program will total the income entered. User entry is disabled in this field.
Enter the amount, if any, of allowable child care deductions.
Enter the amount of allowable medical deductions. This field may be left blank if there are no medical deductions.
Enter the amount of assistance.
The program will display the total of the entered deductions. User entry is disabled in this field.
The program will calculate and display the following information.
The amount of income applicable to the rent. (Income minus exemptions and deductions.)
Shows the Rent Income amount divided by 12 months.
Calculates and displays the tenant payment amount based on information entered.
Click the next button to move on to entering applicant information.
Click the close button to close the program. After the Quick Eligibility Check has been done, the program may be ended here and the applicant information may be entered directly into the Applicant Master.
For a new applicant that has never had their information entered, leave this field blank. The system will assign their number.
For an existing applicant their information may be selected to view/update, or their information may be selected and then deleted using the "Delete Pre-Application" button.
Press the browse button to the right of the field to access the "Pre-Application Browse". If an applicant has had information entered in this area, they will have been given an Application # and their information will be shown in the browse.
Select an existing Pre-App Number and then press this button to delete the applicant information.
Information is only deleted from the wizard. If the applicant is in Applicants (Active), that information will not be deleted.
Select the Application For code the applicant is applying for.
Enter, view or edit the following information for the head of household:
Last Name
First Name
MI (middle initial)
Soc Sec #
Date of Birth
After entering some or all of the requested head of household information, press the "Lookup" button to see if the applicant is or has been an applicant or tenant in the Waiting List or other Horizon housing modules.
Defaults to the current date during an initial entry. For an existing applicant, the date previously entered will be displayed. The date may be changed.
Defaults to the time the wizard was opened for a new applicant. For an existing applicant, the time previously entered will be displayed. Time may be edited.
Clicking the back button will return the wizard back to the "Quick Eligibility Check" page of the wizard.
Click the next button to move to the "General Household" page of the wizard.
Click the "close" button to close the wizard.
Enter or increment to the number of members in the applicant's family.
Enter or increment to the number of dependant members, under the age of 18 or over 18 and a full time student, within the applicant's family.
Enter, view or edit the following information for the applicant's address where they are currently living:

Use the back button to return to the "Welcome" screen of the Applicant Wizard.
Use the next button to continue entering general household information.
Click the close button to close out of the application wizard.
If the applicant receives their mail at an address that is different from the street address used for the present location, enter, view or edit it in the following fields:

When entering a new member, leave the member number field blank. The system will assign the next member number.
For an existing member, select the member number to view or edit.
For a new member, enter the members last name, first name and middle initial or name. Existing member names may be edited.

E - Full Time Student 18+
F - Foster Child/Foster Adult
H - Head
K - Co-Head
L - Live-In Aide
O - Other Adult

Enter or edit the member's social security number.
Select one of the following:

Enter, view or edit the member's date of birth in a mmddyy or mmddyyyy format.
Enter the name of the city where the member was born.
Enter or select the state the member was born in.
Check the box if the member is a United States citizen.

Checkmark the box if race/ethnicity information is entered by the visual observance of the person entering the information and was not volunteered by the member.
Press the "Save Member" button after each member is entered to save them and clear the screen in preparation for entering another member. Members are also saved when the screen is entered through and the member's information drops into the Member window at the bottom of the page.
Press the "Clear Member" button to clear the screen without saving or changing the information on this screen.
Press the "Delete Member" button to delete a selected member.
When a member's information is entered and saved the information is displayed in the "Member" window/grid. The edit box may be checked in the window/grid to bring the member's information back into the entering or edit area. Some information may be changed within the grid.

Enter the following income information for each member directly into the grid:
Member - enter or select a member number. Members who have been entered will be available for selection, go <Back to the previous screen to enter a member who is not available.
Member Name - the selected member's name will be displayed
Class - select the appropriate class for the income
Rate - enter the monthly or unit amount of income
Unit - enter the pricing unit for the rate

Annual - the system may calculate the annual income amount or the annual amount may be entered
Income Source Description - The default description for the selected class will be displayed, but may be overwritten/edited
Place the cursor on the line to be deleted and press the "Delete Line" button to delete the line.
A Confirm box will be displayed as shown below:
The confirmation message should read: "Delete this Line?"
Click the "Yes" button to complete the deletion of the line.
Click the "No" button to cancel the deletion.
Use the back button to return to the previous screen of the Applicant Wizard.
Use the next button to continue the application process.
Click the close button to close out of the application wizard.
Enter the following asset information directly into the grid:
Member - enter or select a member number for the asset
Member Name - the name of the selected member will be displayed
Class - select the appropriate asset class
Description - the default description for the selected class will be displayed and may be overwritten/edited.
Value - enter the value of the asset
Percent - enter the percentage of interest associated with the asset.
Income - The program will calculate the income amount based on the value times the percent. The income may also be overwritten/edited.
Comment - Enter a comment regarding the asset information entered
Place the cursor on the line to be deleted and press the "Delete Line" button to delete the line.
A Confirm box will be displayed as shown below:
The confirmation message should read: "Delete this Line?"
Click the "Yes" button to complete the deletion of the line.
Click the "No" button to cancel the deletion.
Use the back button to return to the previous screen of the Applicant Wizard.
Use the next button to continue the application process.
Click the close button to close out of the application wizard.
Enter the following deduction information directly into the grid:
Class - select the appropriate deduction expense code
Description - The default description for the selected deduction will be displayed and may be overwritten/edited
Rate - enter the amount of the deduction
Unit - select how often the amount is paid from the following;

Day
Weekly
BiWeekly
Semi-Monthly
Monthly
Annual
Annual - the program will compute the amount of the annual income by taking the rate times the unit. The annual amount may be overwritten/edited.
Address1 - Enter the address for the deduction. The deduction code may have a default address entered. Address information only needs to be entered if this deduction's address is different than the default.
Address2 - Enter additional address information if needed. The deduction code may have a default address entered. Address information only needs to be entered if this deduction's address is different than the default.
City - Enter a city for the deduction. The deduction code may have a default address entered. Address information only needs to be entered if this deduction's address is different than the default.
State - Enter a state for the deduction. The deduction code may have a default address entered. Address information only needs to be entered if this deduction's address is different than the default.
Zip - Enter the zip code for the deduction. The deduction code may have a default address entered. Address information only needs to be entered if this deduction's address is different than the default.
Place the cursor on the line to be deleted and press the "Delete Line" button to delete the line.
A Confirm box will be displayed as shown below:
The confirmation message should read: "Delete this Line?"
Click the "Yes" button to complete the deletion of the line.
Click the "No" button to cancel the deletion.
Use the back button to return to the previous screen of the Applicant Wizard.
Use the next button to continue the application process.
Click the close button to close out of the application wizard.
This page of the wizard shows totals for the Income, Assets, and Deduction entered. It also shows Rent Calculations and Income Limit information. Use the Create Applicant button to create the applicant file. The Launch Survey button will start the survey program.
If any of the information shown is incorrect, use the back button to return to the appropriate page of the wizard and correct the entered data.

Clicking the create applicant button will transfer the information entered into the wizard to the Applicant (Active) program. The applicant information in the wizard will be removed and the wizard will close.
Once the information is in the Applicant (Active) program, the applicant may be added to waiting lists.
Clicking the launch survey button will start the Applicant Survey program. When the survey is finished the survey program will close and return the user to the wizard.
Since the "Create Applicant" program closes the wizard, the survey should be performed prior to creating the applicant.
The Survey program is also available in the Applicant Master from the quick link menu. There is also an option to print the survey from the Applicant Master Quick Link menu.

The finish button closes the application wizard. This button does not transfer the applicant into the program. The prospective applicant is available within the wizard for further updating or the information may be used at a later date to create an Applicant.
